    ACKNOWLEDGMENT OF RISKS

    I understand that participation in the 30-Day to 5K Walking Challenge (the “Challenge”) involves inherent risks, including but not limited to the risk of personal injury, cardiovascular events, falls, or other health-related complications. I fully understand that my participation may result in serious injury, illness, or other adverse health consequences, particularly given my diagnosis of Peripheral Artery Disease (PAD). I voluntarily accept and assume all such risks, both known and unknown.

    PHYSICIAN CLEARANCE

    I confirm and represent that I have consulted with my physician or healthcare provider and have received explicit medical clearance to participate in a walking program. I understand that it is my responsibility to adhere to any limitations or modifications my healthcare provider has recommended for my participation.

    INDEPENDENT MEDICAL ADVICE

    I acknowledge that any information, suggestions, or advice provided during the Challenge is educational in nature and not intended as medical advice. I agree that I will not act on any information or advice provided during this Challenge without first consulting with and obtaining explicit consent from my own healthcare team. I understand that the Global PAD Association does not provide medical advice or treatment.

    CONSENT FOR RESEARCH DATA COLLECTION

    I voluntarily consent to allow the Global PAD Association to collect data regarding my participation in the Challenge for research purposes, specifically to determine factors limiting walking ability in PAD patients and whether additional foot exercises and specific walking styles help improve claudication and walking distance. I understand that my personal information will be kept confidential and used only for research purposes in accordance with applicable privacy laws.

    RELEASE OF LIABILITY

    To the fullest extent permitted by law, I hereby release, waive, and discharge the Global PAD Association, its officers, directors, employees, volunteers, representatives, and agents from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in the Challenge, regardless of whether such harm is caused by the negligence of the released parties.

    PARTICIPATION AT OWN PACE

    I understand and agree that I am to participate in the Challenge at my own pace and only to an extent that is safe for me based on my individual condition and limitations. I take full responsibility for monitoring my own physical condition during the Challenge and will discontinue participation if I experience any concerning symptoms.
